Volunteering Forms for REACH

Requirements for Regular School Volunteers:

  • Volunteer Application
  • Signed Volunteer Code of Conduct
  • Copy of CA driver license
  • TB Clearance
  • Finger printing through Livescan

Additional Requirements for Specialty Volunteers

  • Driver Application
  • Driver Vehicile Declaration Page
    • Provide Copies of the following to the school site to add to your Volunteer Application
      • Original DMV Driver Record (Must be done each yaer)
      • Insurance Policy Cover Page indicating the following:
        • Liability $100k per person and $300k per occurence
        • Property Damage 25k
        • Medical Payments 5k

Below are the forms for establishing your volunteer status at REACH.  These only need to be done once at the beginning of the school year.  All parents driving kids on field trips and in-class volunteers will need to fill out forms.
